Abstract: 

Background. Progressive loading protocol is a high-demanding procedure suggested to increase the quality of bone when the implant is inserted in D3 or D4-typed bone. The purpose of this study is to present a case report regarding simultaneously progressive and immediately loading implants in the anterior position of the maxilla. Methods. An electronic search for articles in the English language literature was performed using a systematic search through PubMed/MEDLINE and Google scholar databases with no publication year limit. Besides one patient whose implants in the anterior maxilla (in site of teeth #7, #9, #11) had questionable to poor prognosis who was treated immediate and delayed progressive loading protocol and was followed up for 2 years. Conclusions. Although the number of studies in this field is limited, it can be concluded that the progressive loading protocol can predictably enhance the treatment success in immediate-loaded implants with poor prognosis and when inserted in low quality bone such as the present case.

IMMEDIATE LOADING HAS GAINED A LOT OF ATTENTIONS IN DENTAL REHABILITATION. THE GOAL OF THE PRESENT PAPER IS TO EVALUATE THE STRESS DISTRIBUTION WITHIN THE DENTAL PROSTHESIS AND THE BONE IMPLANT INTERFACE USING FINITE ELEMENT ANALYSIS. A PRECISE 3D MODEL OF HUMAN MANDIBLE AND PROSTHESIS IS EMPLOYED AND SUBJECTED TO IMMEDIATE LOADING. THE RESULTS SHOW THAT THE STRESS LEVELS IN PROSTHETIC COMPONENTS HAVE NOT EXCEEDED THE FAILURE THRESHOLD. THE MAXIMUM STRESS OCCURS AT THE SUPERFICIAL REGIONS OF THE CORTICAL PART AND BUCCAL SIDES OF THE INTERFACE RECEIVE HIGHER STRESS MAGNITUDES.

Abstract: 

To ensure single dental implant stability, there are some approaches. The stabilization of a single‑ piece implant by welding it to a titanium needle allows immediate loading and promotes the final osseointegration. The aim of this case report study was an evaluation of long‑ term clinical outcomes of immediate loading in cases with bone atrophy in the canine region by welding single implant to a titanium needle. In two cases with maxillary atrophic anterior bone, single‑ piece titanium implants were used. As support structure, a Scialom‑ type stabilization titanium needle was used (diameter 1. 2 mm and suitable length to obtain a bicortical positioning). These two elements were joined together using a Mondani intraoral welder. The implants immediately loaded by temporary restorations. The cases were followed for 24 years, and clinical outcomes were assessed. The use of this method, in the cases with maxillary atrophic anterior bone, allows immediate loading with a fixed acrylic prosthesis at the end of the surgical session. Clinical outcome was successful for 24 years. This approach is a successful method that allows immediate loading in atrophic bone.

Abstract: 

When immediate molar implants first were proposed, submerged initial healing and delayed loading were the norm. It is now recognized that some early loading of a nonocclusal nature can stimulate faster osseointegration, although full occlusal loading is still delayed for 3 or more months. Here, we test the hypothesis that earlier occlusal loading of mandibular premolar and molar immediate implants may be possible. In this retrospective case series study, 18 mandibular molar and nine mandibular premolar teeth were atraumatically extracted and immediate implants placed 1–, 2 mm apical to buccal and lingual crestal bone. Periimplant gaps received particular allograft covered with acellular dermal matrix barrier. Healing abutments were placed through puncture points in the membranes to help in stabilizing the latter and to permit nonsubmerged site healing. At 6−, 8 weeks, each implant was evaluated for stability using the Periotest®,device and restored if the Periotest®,(PTV) value seen was negative. Data were analyzed by t test and MannWhitney U at a significance level of P < 0. 05. Retrospective assessment of all 27 implants after 5 years’,period of follow up showed all implants to have survived. Overall mean crestal bone loss was determined to be−, 0. 25 ±,0. 54 mm. Individual mean bone levels for mesial and distal surfaces were−, 0. 24 ±,0. 77 mm and−, 0. 26 ±,0. 72 mm, respectively (P = 0. 78). A statistically significant difference in bone loss between genders was detected. Overall mean probing depth was 2. 09 ±,0. 57 mm. Based on the widely used Albrektsson criteria, the overall survival and success rate was 100%. Immediate implants placed into mandibular premolar and molar extraction sockets and allowed nonsubmerged healing may be ready for restoration at earlier times than previously thought possible.

Abstract: 

Background and Aim: In the last decade, implant surface treatment in order to maintain rapid and improved osseointegration has been considered worldwide. This study reviews methods used to increase osseointegration of dental implants by surface modification techniques.Materials and Methods: A search in PubMed was performed by typing the following words: dental implant surface, implant surface treatment, implants surface modification and 48 relevant articles were extracted from 2000 to 2012.Results: surface characteristics of dental implants such as surface chemistry, thickness of the TiO2 layer, wettability, surface charge, and surface roughness can increase the rate and strength of osseointegration. Many of these methods have been shown to have clinical efficacy (>95% over 5) years.Conclusions: The exact role of the surface chemical characteristics and surface topography in the primary events of osseo integration has not been clarified precisely. In addition, human clinical trials that compare current methods simultaneously in a split mouth design are rare. Many of these studies were in vitro and generalizing the results of these studies to the clinical practice is difficult. Therefore, more clinical trials are suggested.

Abstract: 

Introduction: Maximus dental implants are used when there are limitations like inadequate mesiodistal space. This study was carried out to clinically and radiographically investigate Maximus implants which were installed in the regions of maxillary lateral and mandibular incisors using immediate nonfunctional loading.Materials & Methods: In this clinical trial study, approved by ethical committee of Mashhad University of Medical Sciences, 21 implants for 12 patients (2 male and 10 female) between 18 and 42 years old were inserted. Clinical and radiographic measurements were recorded in recall sessions and analyzed statistically using t.test and ANOVA. Results: After one year of examination, none of the implants failed and the mean bone resorption was 1.06±0.90. The means for pocket depth and plaque index were 1.62±0.3 and 14.57±2.56, respectively. The mean of gingival index after 6 months was 0.21±0.36 and it reached 0 after one year. The mean of bleeding index after 6 month was 0.03±0.085 and after one year it was equal to 0.Conclusion: Considering the very high success rate and low bone resorption around Maximus implants with immediate nonefunctional loading during one year of examination, it appears that using low diameter implants for edentulous regions of small bone width or mesiodistal space is a suitable treatment.

Abstract: 

Over the past decades dental implant with 90-98% success rate has become an integral part of reconstructive dentistry. As the supporting tissues of the implant have different structure and circumstance with natural teeth like less vascularity and different collagen fibers arrangement and amount, dental implants are more susceptible to infection and bone resorption, thereafter an extensive and careful maintenance protocol should be followed. However, the attraction of patient to implant treatment is growing up, the maintenance protocol following treatment is ignored. This neglect or maybe lack of awareness can lead to peri-implantitis, bone resorption and finally implant failure. Although there are some risk factors for periimplantitis like: smoking, history of periodontitis, limited oral hygiene, systemic disease and poor plaque control. The awareness and periodic recall of the patient by dentist (every three months in the first year and every six months thereafter) play a great role in prevention of implant failure. This study is aimed to review the maintenance protocol which is divided into two main category called professional (sonic and ultra-sonic scaler, plastic or Teflon coated curettes, oral irrigation and occlusal adjustment) and home care (manual tooth brush, mechanical tooth brush, dental floss, interproximal brush and antimicrobial). An electronic search for article in English language literature from the past 5 years about implant maintenance was performed using a systemic search process. Nowadays implant treatment has become a main part of dentistry which involves different specialties from periodontology to surgery and prosthesis, the point here is the behavior of the patient after treatment following the principle to keep the implant in a best situation to avoid failure and additional costs.

پس از بازدید کارشناسان از Loading Arm و بررسی شرایط محیطی و انجام تحقیقات بر روی Loading Arm های مشابه، کاتالوگ ها و مرجع های معتبر گروه در قالب استانداردهای مرجع مهندسی و بدون کپی برداری کورکورانه شروع به طراحی کامل نمود که با توجه به داده های موجود و شرایط کارکرد منجر به طراحی تمام مکانیزم ها گردید. طراحی و مدل سازی Loading Arm در قالب نرم افزار Solid Works انجام شد تا بتوان تمام مکانیزم های حرکتی این طرح را به طور کامل شبیه سازی3D نمود و مورد تحلیل قرار داد. طراحی کامل همراه با جزئیات کامل برای ساخت در داخل همراه با شبیه سازی و تحلیل دقیق مکانیزم های حرکتی ازجمله Haydrulic Jac, Swivel Joint و ... در زمان بسیار کوتاه از نتایج این طرح است.
